1. For your first name take the first three letters of (either form) of your first name, and the first two letters of your last name.

2. For your last name, take the first two letters of your mother's maiden name, and the first three letters of the city you were born in.

3. For your honourific, start by taking the last three letters of your last name, reversing them, and adding the make of your first car. Then put the word of.

4. Now put the name of the last medication you took.
